Declaration

If an incident has already been triggered before the inception date of your insurance policy which will also be deemed to be the date you have been accepted into membership of the PDA and the PDA Union then your policy will not cover such events or the consequences of those events. PDA and the PDA Union will be able to help to some extent but this will be subject to certain conditions which will be explained to you by our professional or legal advisors.

However it is in your interests to inform us of any of the following if we are to be of any help to you should there be repercussions or consequences of your actions. We need to know immediately if:

  • A claim has been made against you in the past for medical negligence
  • You have been prosecuted or received a caution for a criminal act
  • You have been subjected to any formal investigation by a regulatory body (GPhC, PCO, University FtP Committee etc.)
  • You have any health related problem that could affect your fitness to practice.
  • You have been dismissed by an employer for breach of trust or dishonesty.

Even if these events occurred before your membership was incepted informing us of any such incident will not affect the support we give you but failure to inform us at this juncture will prejudice your position and if there are any consequences you may invalidate your membership benefits.

A more comprehensive description of the policy terms can be found online and you are strongly advised to read the policy wording in full.
